Imaging Records Access
Access Your Imaging Records Through Clearpath
With a Clearpath account, you can access your imaging records without needing a CD. This secure platform keeps your information protected while making it conveniently accessible to you at any time, from any device.
HOW TO GET STARTED
1. Create a Clearpath account
Enter your name, date of birth, email address, and time zone.
2. Log into Clearpath
Using the password sent to you, you will now be able to log in to the Clearpath system.
You will be asked to create a new password.
3. Upload a copy of your photo ID
Before images can be released, you must verify your account by uploading a copy of your photo ID. This will only be required once, before your first records request is made.
To add your ID, click on the MY IDENTITY tab,
HOW TO REQUEST YOUR RECORDS
- Once logged into Clearpath, select the “Request Record Tab.”
- To request a record, select the “Request Record” button in the bottom right corner.
- Select “Record Custodian” (Provider Location).
- If you do not see Atlantic Medical Imaging’s name listed, please reach out to help@myclearpath.com.
- Select “records to request,” body part, and date range.
- Agree to the terms and conditions and submit.
Once submitted, Atlantic Medical Imaging will review and process it. Once your request is processed, if approved, you will see your images under the “My Records” tab.
